Pure Clarity Coaching Agreement
Parties: The agreement is between Pure Clarity, Sarah Siegel-Kolodziejski and ‘Client’ The
Coaching Package: 1:1 Coaching Monthly Package. 4 x 60 min sessions with me as your coach, mentor, and guide. Your Life. Lived Well. 12 Week Program. 10 x 1:1 Sessions, 1 Wellness Session and 1 Breathwork Session. Email and telegram support to ask questions, get challenges solved, or celebrate your wins.. Assignments and exercises to implement next steps. An additional fee is assessed for in person sessions.
Estimated start and end date: Mutual agreement between Client and Pure Clarity
Nature of the Coaching Relationship:The client/coach relationship is a partnership. In no way is the coach the authority in the client’s life. You, the client, commit to progress and growth and co-create the agenda for each call with your engaged learning and doing. You are responsible for decisions made and actions taken as a result of the coaching calls. How to Prepare for a Coaching Session: Each of our meetings will expand on one another as you commit to 4 (monthly coaching package) or 12 sessions (Your Life. Lived Well. Program). Depending on Program or Package selected.
In order to work most efficiently and achieve the greatest success, here is how we agree to prepare for each session: Each session will end with an action item for you to complete before our next session. This may include a reflection, question to answer, exercise, or implementation step or activity. Before each session (24 hours in advance), you may be asked to complete an ongoing pre-call form describing your successes, challenges, and focus areas.
To prepare for our call, you will decide on what area you’d like to focus on. If you are unsure, I can help you determine where my help will be the most effective.
Coaching is not a “done for you” relationship, but rather a co-creative one where your biggest results will result from your own work. In order to be successful, try making notes about any challenges or opportunities for growth and keep a list of questions for our next meeting. Client Agreement. I understand that coaching is not a form of therapy or outpatient support, although coaching may be used in conjunction with therapy.
I understand that my coach, Sarah Siegel-Kolodziejski, is not a doctor and thus not able to give medical (mental or physical) advice from a medical standpoint.I understand that I may stop our coaching sessions at any time, but that will not allow for a refund. I understand that my coach will be sharing proprietary information and intellectual property with me. While I may be inspired, I understand that this material is copyrighted and legally protected. I understand that my coach’s qualifications are as follows: Certified Hypnosis, NLP, Mindset, Success, Alcohol Free Living Practitioner.
I understand that what we discuss is confidential unless I indicate, in any way, that I would harm myself or others then the proper authorities will be notified.I understand that my coach does not control the direction of my life.I understand that if I need to cancel an appointment, I will give a 48 hour notice. My appointment will be rescheduled or tacked on at the end of the term. After 3 appts rescheduled within a 48 hour time frame, the session will be forfeited.
I understand that calls may be rescheduled and the 12 week package may extend over 1 month, however I have up to 4 months from the time of purchase to use my sessions before my sessions expire. I understand that I will show up for our sessions on time and be prepared.
What to include in the T&C document
Generally speaking, T&C often address these types of issues: Who is allowed to use the website; the possible payment methods; a declaration that the website owner may change his or her offering in the future; the types of warranties the website owner gives his or her customers; a reference to issues of intellectual property or copyrights, where relevant; the website owner’s right to suspend or cancel a member’s account; and much, much more.
